Epidemiologic investigation on antibiotic use using defined daily dose

Abstract
Inappropriate use of antimicrobial agents plays a significant role of increasing the antibiotics-resistance, so results in reducing antibiotic efficacy, and augmenting the burden of disease and medical cost. Although it s very important to use antibiotics with appropriateness, there has been no proper collection of data nor there has been proper studies done in order to analyse appropriate usage of antibiotics in Korea. Therefore, this study was conducted to suggest several interventions for the appropriate usage of antibiotics by quantifying the antibiotics use. This study was to calculate the volume and epidemiologic characteristics of antibiotics consumption in the national level using health insurance claim data. So this study was investigated DDD (Defined Daily Dose) of WHO as a measure of antibiotic use as the type of patients characteristics (gender, age, region, disease), provider characteristics (medical institution type, department). According to the analysis of antibiotic consumption using 2004 health insurance claim data of HIRA (Health Insurance Review and Assessment Service), it showed that the usage of antibiotics in 2004 was calculated as 23.62 DDD/1000 inhabitants/day, which is 6th highest compared to other OECD countries. And the proportion of patients who received a prescription for antibiotics is 30.3%. The most significant point is that 47.6 DDD/1000 inhabitants/day was shown for those who were 6 years of age and under, which is much higher than other age groups. Also, for the clinics outpatient group, the volume of antibiotics accounted for about 75.6% of the total volume with 17.63 DDD/1000 inhabitants/day. We concluded 24 adults per 1000 inhabitants everyday consume antibiotics nationally. This study would provide basic statistics for health policy of antibiotics management.
